INFORMED CONSENT PROCESS:

Briefly stated, each person and their family are allowed to take the consent with them in order to discuss with friends, family or Doctor. Upon sufficient review, that patient is provided the opportunity to ask questions and share concerns with PI or SUB-I. Once the questions are answered to the patient’s satisfaction, they sign the informed consent in front of staff. Authorized staff  also witness and sign the document and initial each page. The Principal Investigator also reviews and signs the document. The patient is given a copy of the signed consent. Should there be official IRB changes to a consent document, then all patients repeat this process and sign the new consent at the earliest possible moment.
